Abnormal Response of Post-Stress
Left Ventricular Ejection Fraction in
Patients With Inducible Ischemia

Introduction: Gated myocardial perfusion imaging allows the evaluation of the left ventricular function (LVF) at rest and during post-stress. Recent publications have suggested that in patients with inducible ischemia the post-stress LVF does not always represent the resting LVF.

Objectives: To evaluate LVF [ejection fraction (EF), end diastolic volume (EDV) and end systolic volume (ESV)] at rest and during post-stress in patients with stress-induced reversible defects.

Methods: One hundred and four patients underwent single-day stress-rest Gated-SPECT with 99mTc MIBI. Parameters of LVF were calculated using a QGS (quantitative Gated-SPECT) program. All patients performed a symptom-limited exercise treadmill test and images were recorded within 10 minutes after completion of the exercise with a dual-head camera. Patients were divided in group 1: (n= 50; 60±10 years) inducible reversible perfusion defects and group 2: (n= 54; 57±12 years) normal perfusion and normal LVF at rest as control group.

Conclusions: Patients with stress-induced reversible defects reveled a decrease in EF with a significant increase in ESV immediately after exercise. In contrast, in patients with normal perfusion and normal LVF, EF significantly increased and was associated to a decrease in ESV. Thus, the impaired response of LVF during post-stress in patients with inducible ischemia could be attributed to the presence of myocardial stunning.
